Payment due at the time of service if not billing Insurance (per 50-55 minute hour) $100.00

If you would like to use insurance to help pay for therapy please call your insurance company prior to making an appointment to see

what your benefits cover.

   -What are my outpatient mental health benefits for in and out of network providers?

   -Do I need preauthorization? If yes, how do I obtain it?

   -Is there a limit to the number of sessions I can bill to my policy in a calendar year?

   -What is my co-pay likely to be?

   -Please bring your insurance card to your appointment. Otherwise, you are welcome to pay for services         out of pocket.
